Position Summary:

 Reporting to the Manager, Health Disciplines Practice within Professional Practice, the Spiritual Care Practitioner is responsible for providing professional, compassionate emotional and spiritual care to patients, their families and staff at Trillium Health Partners. 

Job Description:

  • Provides spiritual and supportive care to patients, families and staff through spiritual assessment, supportive intervention, and counselling
  • Acts as a consultant on end-of-life decision-making and ethical issues, and as a resource to support care teams in understanding, respecting, and integrating faith traditions and practices into holistic health care
  • Participates as a member of the interprofessional team to enhance continuity of care by attending interprofessional rounds and other patient consultations when requested
  • Promotes best practices in religious and spiritual care, including end-of-life care, grief and bereavement support
  • Establishes positive working relationships with members of the health care team, leadership, community faith leaders, volunteers, patients, families and visitors
  • Develops and facilitates education sessions for staff and volunteers
  • Engages with community resources to provide appropriate spiritual and religious care for patients of all faiths
  • Performs other duties as assigned and that align with THP’s Strategic Plan 

Qualifications:

  • Master’s degree in Theology, Divinity, Spiritual Care, Pastoral Studies, or equivalent required
  • Registered with the College of Registered Psychotherapists of Ontario (CPRO), or in the process of registering
  • Preference will be given to candidates who are Certified Members of the Canadian Association for Spiritual Care (CASC), or those who have completed Advanced CPE training (4 units of CPE or combination of equivalent education and relevant experience preferred)
  • Endorsement and in good standing within their faith group 
  • Experience working with diverse faith communities with demonstrated knowledge of various faith practices and traditions
