Position Summary
The Diabetes Management Centre (DMC) is one of Ontario's largest adult outpatient diabetes programs, providing comprehensive diabetes education and management services to adults living with diabetes. The program supports individuals living with Type 1 and Type 2 diabetes, and diabetes in pregnancy, through an interdisciplinary team of Nurse Practitioners, Registered Nurses, Registered Dietitians, Social Workers, Kinesiologists, Pharmacists, Endocrinologists, and Administrative Professionals.
We are seeking an innovative and collaborative Nurse Practitioner to join our Diabetes Management Centre team. This position will play a key role in advancing rapid access diabetes care initiatives, including expedited assessment and management of patients referred from the Emergency Department (ED) and Urgent Care Centres (UCC). The role will also support the development and implementation of emerging care pathways for patients experiencing urgent diabetes-related issues in other specialty populations, such as oncology and nephrology.
The Nurse Practitioner will function in an advanced practice role, utilizing a high degree of autonomy and clinical expertise to assess, diagnose, treat, prescribe, order and interpret diagnostic tests, and manage diabetes-related conditions. The NP will work collaboratively with endocrinologists, primary care providers, and the interdisciplinary diabetes team to optimize patient outcomes, and improve timely access to specialty diabetes care.

- Provide assessment and management for patients with diabetes and significant comorbidities, including cardiovascular disease, kidney disease, malignancy, mental health concerns, and frailty.
Program Development and Innovation
- Collaborate with endocrinology, inpatient and outpatient care teams, and operational leaders to improve transitions from acute care to ambulatory diabetes services and enhancement of expedited referral pathways.
- Participate in the design and development of emerging diabetes pathways, including urgent assessment models supporting patients with diabetes or treatment-related hyperglycemia.
- Identify opportunities to reduce ED visits and hospital utilization through proactive outpatient management and timely intervention.
Interprofessional Collaboration
- Work closely with endocrinologists, diabetes educators, primary care providers, specialists, pharmacists, social workers, and administrative teams.
- Support shared care models and provide consultation to members of the interdisciplinary diabetes team.
- Participate in case conferences, clinical rounds, quality improvement initiatives, and program planning activities.
- Serve as an advanced clinical resource within the Diabetes Management Centre
Quality, Education and Leadership
- Contribute to quality improvement, program evaluation, and service redesign initiatives.
